We require a copy of your requisition before booking. Your doctor’s office can fax it to us or you can email it to info@nsmi.ca. Please allow us 4 hours to process your request.
Our staff are available to schedule your appointment Monday to Friday, 8:00 AM to 5:00 PM.
Please note: all examinations require a completed requisition form signed by your doctor.
If you need to cancel or reschedule your appointment, we require 48 hours notice or a missed appointment fee of $75.00 will be charged.
